What BUN Actually Is

Blood urea nitrogen measures the nitrogen-containing portion of urea circulating in the bloodstream, a waste product the liver manufactures whenever the body breaks down dietary or muscle protein for energy and cellular repair. Ammonia is a natural byproduct of that breakdown process, but ammonia is quite toxic in even small concentrations, so the liver converts it into urea specifically because urea is far safer and easily dissolved in water, allowing the kidneys to filter it out efficiently and excrete it in urine on an ongoing basis. A rising BUN level can reflect reduced kidney filtering ability, a high-protein diet, gastrointestinal bleeding, or simple dehydration, since a more concentrated bloodstream naturally raises the measured value even when true kidney function itself remains completely normal and unaffected by any disease.

How BUN Is Measured

Modern laboratories measure BUN using an enzyme called urease, which breaks urea down into ammonia and carbon dioxide gas; a second enzymatic reaction then consumes a light-absorbing compound called NADH in direct proportion to how much ammonia was produced, allowing a spectrophotometer to calculate the original urea concentration with real precision and speed. This automated enzymatic approach has almost entirely replaced the slower manual manometric techniques Van Slyke originally developed decades earlier, which required painstakingly measuring the exact volume of gas released during the underlying chemical reaction by hand, one sample at a time, in a process that could take considerably longer to complete.
