The History of eGFR

Estimating kidney function without invasive testing has a long, decades-long history, beginning with Donald Cockcroft and Henry Gault's 1976 formula based on serum creatinine, age, and body weight, a genuinely useful clinical starting point for its era of medicine and diagnosis. A more statistically rigorous approach arrived in 1999, when researcher Andrew Levey and colleagues published the Modification of Diet in Renal Disease, or MDRD, study equation, which offered meaningfully improved accuracy over the older formula across a much larger and more diverse patient population studied nationally. What eGFR Actually Is

Glomerular filtration rate represents how many milliliters of blood the kidneys' tiny filtering units, called glomeruli, can thoroughly clean every single minute of every single day without pause, widely considered the single best overall measure of kidney function available to doctors anywhere in medicine today. Measuring this directly requires injecting a specialized marker substance into the bloodstream and carefully tracking its clearance over several hours in a controlled clinical setting, a process too impractical and invasive for routine, everyday clinical use in most ordinary patients seen in a busy clinic. eGFR instead estimates this same value using a person's readily available blood creatinine level combined with their age and sex, giving doctors a fast, reliable approximation without ever needing the more cumbersome direct testing method described above at all.

How eGFR Is Measured

Rather than being measured directly at all, eGFR is calculated using the current CKD-EPI 2021 equation, a mathematical formula that combines a person's serum creatinine result with their age and sex to produce a single estimated number automatically and instantly every single time. Most modern laboratory computer systems calculate this instantly and print it directly alongside every routine creatinine result on the report itself, so patients typically never need to request it as a separate additional test on its own accord. Some laboratories now also incorporate cystatin C into an alternative, sometimes even more accurate version of that very same underlying equation for select patients who genuinely need that extra precision.
