What FSH Actually Is

Follicle-stimulating hormone is produced by the anterior pituitary gland, a small structure at the base of the brain, and travels through the bloodstream to directly stimulate the growth of ovarian follicles in women and sperm production within the testes of men. Its release is carefully controlled by gonadotropin-releasing hormone from the hypothalamus, and it participates in a tightly regulated feedback loop involving estrogen, progesterone, and testosterone, all of which signal back to the brain to fine-tune exactly how much FSH gets released at any given moment. How FSH Is Measured

This modern approach replaced the far more laborious and considerably less precise bioassays used decades earlier, which relied on directly observing physical changes in laboratory animals injected with a patient's blood or urine sample. Because FSH naturally fluctuates throughout the menstrual cycle, doctors typically specify a particular day of the cycle, most often day three, for blood collection to ensure genuinely consistent and clinically meaningful results across different testing occasions.

Selmar Aschheim and Bernhard Zondek deserve enormous credit for their pioneering, foundational work connecting pituitary and reproductive hormones to observable, measurable biological effects, research that essentially launched the entire field of clinical reproductive endocrinology as we understand it today. Herbert Evans and his Berkeley colleagues later refined that early foundation considerably, carefully separating FSH from luteinizing hormone as two genuinely distinct pituitary hormones with importantly different physiological roles throughout the reproductive system.
